Application Submission
• Eighteen (18) stapled, double-spaced, typed copies are required.
• Proposals are limited to eight (8) pages.
• Title pages, binders, folders, and special covers should not be used.
• Each proposal must include the following:
COVER SHEET
The Proposal must begin with the typed Cover Sheet provided by the Foundation. When completed, this Cover Sheet becomes the first of the eight pages of the Proposal.
TEXT
The text of the proposal (no more than four [4] single-sided, 8.5 x 11 inch pages, numbered pages 2-5) should include the following:
a) Brief history of the association.
b) Brief description of the association's present activities.
c) Needs for which the grant is being requested.
d) How the association plans to meet the needs.
e) Amount of money being requested.
f) How the matching monies will be raised.
FINANCIAL SECTION
The Financial Section of the proposal (no more than [3] 8.5 x 11-inch pages, numbered pages 6-8) should include the following:
a) The association's operating budget for the year of application.
b) The Statement of Activities taken from the audit of the year immediately preceding application. (May be printed on both sides of the page, if necessary)
c) The Statement of Financial Position taken from the audit of the year immediately preceding application. (May be printed on both sides of the page, if necessary)
SUPPORTING MATERIALS
In addition to, and not attached to this proposal, please include a single copy of each of the following in the grant submission package:
• IRS form 501(c)(3). All applicants must supply documentation of this IRS status. Any substitution of another tax-exempt status must first be verified with the Executive Director of the Foundation.
• One complete copy of the AUDIT of the year completed immediately preceding application.
• A set of relevant brochures, promotional materials, etc.
